City of Yakima Grievance Procedure

Under the Americans with Disabilities Act This Grievance Procedure is established to meet the requirements of the Americans with Disabilities Act of 1990 (“ADA”). It may be used by anyone who wishes to file a grievance alleging discrimination on the basis of disability in the provision of services, activities, programs, or benefits by the City of Yakima. The City of Yakima’s Administrative Policy governs employment-related complaints of disability discrimination.

The grievance should be submitted in writing, preferably on the City Grievance Form, and contain information about the alleged discrimination such as name, address, phone number of grievant and location, date, and description of the problem. Alternative means of filing grievances, such as personal interviews or a tape recording of the complaint, will be made available for persons with disabilities upon request.

The complaint should be submitted by the grievant and/or their designee as soon as possible but no later than 30 calendar days after the alleged violation to:

City of Yakima
Connie Mendoza, ADA Coordinator
129 N 2nd Street
Yakima, WA 98901
(509) 575-6090

Within 30 calendar days after receipt of the complaint, a City designee will meet with the grievant to discuss the grievance and possible resolutions. Within 15 calendar days following that meeting, the ADA Coordinator will respond in writing, and where appropriate in a format accessible to the grievant. The response will explain the City’s position and offer options for substantive resolution of the grievance.

If the response by City staff does not satisfactorily resolve the issue, the grievant and/or their designee may appeal the decision by letter or email within 15 calendar days after receipt of the response to the City of Yakima’s City Manager.

Within 30 calendar days after receipt of the appeal, the City Manager will meet with the grievant to discuss the complaint and possible resolutions. Within 15 calendar days after that meeting, the City Manager will respond in writing, and, where appropriate, in a format accessible to the grievant, with a final resolution of the grievance.

All written complaints received by the ADA Coordinator or appeals to the City Manager and responses from these two offices will be permanently stored by City of Yakima.

Please see ADA Grievance Form for physical or electronic submission to the ADA Coordinator.  If you need assistance in completing this form please reach out to the ADA Coordinator, Connie Mendoza, Connie.Mendoza@yakimawa.gov.
